Help schools qualify for extra funding - completing the income verification form can add support to your school's academic and extracurricular programs. Our schools receive extra funding from the State to support academic and extracurricular programs. By completing a quick form, your child may be eligible for reduced priced home internet, county bus passes, free or reduced breakfast and lunch next school year, and more. Even if your child may not take advantage of these services, completing this form will allow schools to qualify for grants, receive free transportation on field trips, and help qualify for more funding for extracurricular enrichment programs. It is important for your child and the entire school community. Please take a few minutes to complete a form over the phone by calling Diane Nunes at (408) 558-4203.
Or you can print, Household Income Data Collection – Union School District 2020-2021  complete, and return to your child's school as soon as possible.
